One Time Passwords are an extra security code you type when you login to the SEGA ID management page. This extra security code is given to you on your mobile phone. Some applications may be only in Japanese. We do not suggest doing this on your own without some Japanese knowledge.
Android:
iPhone
Basic Phone / WAP
OTP are just an extra layer of security against those who have acquired your username and password for PSO2 and barring them access to your personal information. The One Time Password Management Page should be used only by those who know what they are doing.

Err, about the One Time Password, where's the app we're supposed to download for our phone? Will it work if our phones are US (I assume non-jailbroken phones are not region free)?

The app you're looking for is SecureOTP, it is on GooglePlay if you use an Andriod Phone and I-SecureOTP for iPhones. Ensure it's the app by ThirdNetworks before you install.

I don't suggest doing this blindly but the Google App is https://play.google.com/store/apps/details?id=jp….

Is the OTP system only used for logging on to your Sega ID account, and not to the game?

I've installed the SecureOTP app myself. I don't think you don't need to be in a specific region, but you must install the right app, and create an account on your phone before you can submit its serial on the PSO2 website.
Also, once you submit the serial it cannot be undone, so be careful.
Don't install the SecureOTP For Biz version btw; it's not compatible.

Whoops, double negative there in my last comment, but most people got what I meant nonetheless.
Also, I can't say for certain, but the iPhone and web app should function in exactly the same way. I'd make a screenshot guide, but I can't exactly un-do the OTP stuff.
